The Student-Athlete Medical History Form must be filled out by all new student-athletes, before taking the pre-season physical. After filling out the form, email a copy to your team's athletic trainer (see below). You will be able to sign the form on the day of the physical.

NOTE: You must save the PDF form to your computer before filling out any of the information. The information will only be saved if you open the PDF in Adobe Acrobat. If you fill the form out in your web browser, all the information will be lost.
